Work Permit

Print a work permit application. Work permit applications are also available in the main office. Please have your parent/guardian and your employer fill out and sign the application. Bring the completed form along with a photo ID (could be school ID or driver's license) to the Main Office to Ms. Agustin before school, during lunch or after school. Forms will not be accepted during Tutorial or class time. 

 

Ms. Agustin's work permit hours:

Monday-Friday: 7:30am-8:30am, during lunch, and after school until 4:00pm.

 

If you have work experience with our Work Experience teacher, work permits need to be processed with this teacher.
